The 2015 Lagoon General Permit (LGP) Notice of Intent (NOI) is now available for LGP issuance. The downloadable document is an Adobe Acrobat form with fillable fields. Upon completion of the NOI, please print and have authorized responsible facility official sign the NOI, or you may digitally sign the form and submit it electronically to our offices. Signed electronic copies can be submitted directly to VelRey Lozano (lozano.velrey@epa.gov).

The Lagoon General Permit was finalized and public noticed in the Federal Register on December 18, 2015. The final permits are effective as of January 1, 2016.

Whole Effluent Toxicity (WET) Laboratory Reporting form

The Region 8 NPDES WET Laboratory Reporting form is a Microsoft Excel based form for use by WET laboratories to report bench data. This suggested form contains macros, drop-downs, and fillable fields to enable laboratory personnel to more easily complete WET reporting needs for NPDES permittees. The form is protected to enable the fields to properly function and there is no password set if laboratories would like to further customize the template.
