The CCR Certification Form (revised January 2015) can be used to certify that community water systems in Wyoming or on Tribal Lands in EPA Region 8 have completed and distributed their annual Consumer Confidence Report (CCR) or water quality report. Systems are required to submit this signed form to EPA Region 8 by October 1st of each year.
