What Class I Substance Reporting Forms Should I Complete?

  1. Producer Quarterly Report: Any entity that produces Class I substances. Entities that do not produce Class I substances (such as second-party transformation or second-party destruction facilities) should not use this form.
  2. Importer Quarterly Report: Any entity that imports Class I substances from outside of the United States or U.S. Territories. Shipments of Class I substances deriving from U.S. Territories (Puerto Rico, U.S. Virgin Islands, American Samoa, Guam, or Northern Mariana Islands ) are not considered imports.
  3. Exporter Annual Report: Any entity that exports Class I substances outside of the United States or U.S. Territories. Shipments of Class I substances to U.S. Territories (Puerto Rico, U.S. Virgin Islands, American Samoa, Guam, or Northern Mariana Islands) are not considered exports.
  4. Notification of Transfers of Article 5 Allowances, Essential Use Allowances, or Essential Use Chlorofluorocarbons (CFCs): Any entity that transfers Article 5 allowances, essential use allowances, or essential use CFCs to another entity.
  5. Essential Use Allowance Holders and Laboratory Supplier Quarterly Report and Essential Use Allowance Holder Annual Report: Essential use allowance holders and distributors of laboratory supplies.
  6. Laboratory Certification Report: Distributors or laboratory customers purchasing Class I substances under the global laboratory essential use exemption. This form certifies to the seller of laboratory use Class I substances that the buyer will use the substances solely for laboratory applications and will not resell or use the substances in manufacturing. EPA does not collect this report. Purchasers of Class I laboratory substances provide this report to sellers of Class I laboratory substances.
  7. Second-Party Destruction Annual Report: Any entity that destroys Class I substances that they did not produce. Second-party destruction facilities should not use the Class I Producer Form for reporting destruction amounts; only producers fill out that form.
  8. Second-Party Transformation Annual Report: Any entity that transforms Class I substances that they did not produce. Second-party transformation facilities should not use the Class I Producer Form for reporting transformation amounts; only producers fill out that form.
