Form 3520-12: Fuel manufacturer notification for motor vehicle fuel

This form is used to register fuels under Title 40 CFR Part 79 - Fuel and Fuel Additive Registrations (FFARs).

Important notes:

  1. Checking “Unleaded Regular Gasoline” in Item 5 will include “Unleaded Premium Gasoline” and “Unleaded Midgrade Gasoline” unless you choose to submit a form for each.

  2. Additives listed in Item 7 must be registered.
