Resources for owners/operators responding to the Information Collection request for the Oil and Gas Industry 

EPA is providing several resources to assist ICR recipients in filling out their surveys:

ICR Forms

Part 1 Operator Survey (xlsx) (1 MB)  (Updated 11/17/16 to fix drop-down issue) - If you have been asked to respond to the Part 1 Operator Survey, download it here. You may submit it electronically or by hard copy. Instructions for submitting responses are included in your ICR letter.

• If you prefer to submit the survey by hard copy, please use this form.
• Part 1 Operator Survey (Large Form) (xlsx)(1 pg, 4 MB)  - Use this form to respond to the Part 1 Operator Survey if you are an owner/operator of large numbers of well and/or centralized production surface sites.

Part 2 Gathering and Boosting Tool (xlsx) (326 K) - If you are an owner/operator of a gathering and boosting compressor station and pipeline facilities, use this spreadsheet to enter basic information about your facilities. The tool will then randomly select about 10 percent of your facilities for Part 2 facility survey reporting. 

Part 2 Selected Production Well List (xlsx)(912 K)  - This spreadsheet lists the oil and natural gas production wells for which the Part 2 Facility Survey will need to be completed. If you are an owner/operator of one or more production wells, use this spreadsheet to determine whether you must complete the Part 2 Survey and if so, for which wells. 

Part 2 Facility Survey (xlsx) (482 K) - This spreadsheet will show you the information you will be asked to provide if you have been asked to fill out the more detailed facility survey. To make it easier for you to respond, EPA is using the electronic Greenhouse Gas Reporting Tool (e-GGRT) to collect the data and information from the facility survey.
