Property Approval
- Due prior to use of grant funds on specific property (PO approval required).
- Submit to your PO.
- Instructions for filling out form
- Questionnaire for Hazardous Substance sites(5 pp, 69 K)
- Questionnaire for Petroleum Sites(5 pp, 71 K)
Quality Assurance Project Plan (QAPP) Template
- Quality Assurance Project Plan (QAPP) Template
- Due at least 30 days prior to the initiation of the first project activity involving the collection of environmental samples (PO approval required).
- Submit to your PO.
All Appropriate Inquiries (AAI) Reporting Checklist
- Signed checklist must be submitted to PO with each completed Phase I report.

Site Specific Sampling and Analysis Plan (SAP) Template
- Site Specific Sampling and Analysis Plan (SAP) Template
- Due at least 30 days prior to the initiation of each site sampling and analysis investigation (PO approval required).

Reporting
- Quarterly Progress Report Templates

- Cleanup grant recipients are encouraged to submit via ACRES Reporting (see below)
- ACRES Reporting
All grantees are strongly encouraged to submit Property Profile forms via ACRES (Assessment, Cleanup, and Redevelopment Exchange System)- Current Users—Login to ACRES
- New Users—Register for ACRES by clicking "One Time Registration"
If you are unable to submit the Property Profile form via ACRES, you may fill out, print, and submit the following form to your PO.
- MBE/WBE Reports (under old MBE/WBE Program for grants awarded prior to May 2008)
- Due on an annual basis by October 30th of each year
- Submit to Hana Hyland with a courtesy copy to Tanya Thomas [links to MAILING ADDRESS at end of doc]
- Preferred method is to email a pdf file to R3_MBE-WBE_Reports@epa.gov with a courtesy copy to your PO and the grants specialist (thomas.tanya@epa.gov).

Closeout
Closeout reports must be submitted no later than 90 days after the end of the cooperative agreement period of performance. The following must be submitted:
- Final MBE/WBE Reports (to Cynthia Burrows and Tanya Thomas)
- Final Lobbying and Litigation Certification (to Tanya Thomas)
- Final Progress Report (to your project officer)
- Final ACRES Reporting
- Final Federal Financial Report (To Las Vegas Finance Center)
